    Innan jag tatuerar mig hos Levis Tattoo bekräftar jag följande:

    Jag har fått fullständig information om riskerna med tatuering, inklusive infektion, ärrbildning, allergiska reaktioner och andra kända eller okända komplikationer. Jag förstår riskerna och accepterar fullt ansvar för eventuella följder under och efter behandlingen.

    Jag ger mitt fulla samtycke till behandlingen och avsäger mig rätten att rikta krav mot artisten eller studion för skador som kan uppstå i samband med tatueringen, oavsett orsak.

    Jag har haft möjlighet att ställa frågor och har fått tillfredsställande svar.

    Jag har fått och förstått skötselråd samt förbinder mig att följa dessa. Jag är medveten om att bristande efterlevnad kan leda till komplikationer och eventuella extra kostnader.

    Jag är inte påverkad av alkohol eller droger och genomgår behandlingen frivilligt.

    Jag har inga medicinska tillstånd (såsom blödningssjukdom, epilepsi, diabetes, hjärtfel, smittsamma blodsjukdomar, hudsjukdomar m.m.), är inte gravid eller ammar, och tar inte blodförtunnande mediciner. Jag informerar tatueraren om relevanta hälsotillstånd, inklusive om jag har lätt att svimma.

    Jag ansvarar själv för stavning, innehåll och betydelse av vald design.

    Jag är medveten om att färg och resultat kan påverkas av min hudton samt att tatueringen kan blekna över tid.

    Jag förstår att en tatuering är en permanent förändring av huden och att borttagning är kostsam och kan ge ärr.

    Jag ger mitt tillstånd att studion får fotografera och publicera bilder av tatueringen (om inget annat skriftligen avtalats).

    Jag bekräftar att jag tagit del av informationen i god tid, förstått innebörden och att detta är ett juridiskt bindande avtal.

    Jag intygar att jag är minst 18 år, har uppvisat giltig legitimation och är vid fullt medvetande när jag undertecknar detta dokument.

    Genom min underskrift bekräftar jag att jag har läst, förstått och accepterat samtliga villkor.

    Please read all terms carefully before signing.

    Before getting tattooed at Levis Tattoo, I confirm the following:

    I have received full information regarding the risks associated with tattooing, including infection, scarring, allergic reactions, and other known or unknown complications. I understand these risks and accept full responsibility for any consequences during and after the procedure.

    I give my full consent to the procedure and waive the right to make any claims against the artist or the studio for injuries or damages that may occur in connection with the tattoo, regardless of cause.

    I have been given the opportunity to ask questions and have received satisfactory answers.

    I have received and understood the aftercare instructions and agree to follow them. I understand that failure to follow these instructions may result in complications and additional costs.

    I am not under the influence of alcohol or drugs and am undergoing this procedure voluntarily.

    I do not have any medical conditions (such as bleeding disorders, epilepsy, diabetes, heart conditions, blood-borne diseases, skin conditions, etc.), I am not pregnant or breastfeeding, and I am not taking blood-thinning medication. I will inform the tattoo artist of any relevant health conditions, including if I am prone to fainting.

    I am solely responsible for the spelling, content, and meaning of the chosen design.

    I understand that color and final results may vary depending on my skin tone and that the tattoo may fade over time.

    I understand that a tattoo is a permanent alteration of my appearance and that removal is costly and may cause scarring.

    I give permission for the studio to photograph and publish images of my tattoo (unless otherwise agreed in writing).

    I confirm that I have received this information in sufficient time, have read and understood it, and acknowledge that this is a legally binding agreement.

    I certify that I am at least 18 years old, have presented valid identification, and am of sound mind when signing this document.

    By signing, I confirm that I have read, understood, and accepted all terms and conditions.
